I on behalf of myself, members of my family, my heirs, executors, administrators and assigns, hereby forever release, discharge and hold harmless Sunnyvale Martial Arts Academy, its employees, owners, representatives and agents (taken as a group, “SVMAA”) for any injury, loss or damage to my person or property howsoever caused (called “Damage” in this document), arising out of or in connection with my (or my child) taking part in martial arts classes, whether in person on by video connection (for example, on Zoom), special events, seminars and activities and even though that Damage may have been contributed to or occasioned by the negligence of SVMAA. 

I hereby fully assume responsibility for all liability and all risk of injury or loss sustained which may result from activities at the SVMAA school. I also recognize that when I or my child take(s) classes via Zoom, I am, or my child is, doing so in my home, my yard or at some other location that I have chosen other than the SVMAA school, where SVMAA does not control what flooring, furnishings or room dimensions are in the area where I decide to train, or decide to have my child train. Therefore, I assume responsibility, and release SVMAA, for any Damage to me, my family members or my property that occurs when I or my child take(s) classes via Zoom or Online classes, whether at home, in my yard or another location of my choosing. 

By selecting the terms and conditions check box,  I release SVMAA, all instructors, and all other students of Sunnyvale Martial Arts Academy from any and all liabilities for any type of injuries or loss sustained while at the SVMAA school or when I train in classes or activities conducted on Zoom or another video-conferencing application. The undersigned also states that he/she (or his/her child who is a student SVMAA) is in good physical condition. In the event of an emergency that takes place at the SVMAA school or other location chosen by SVMAA, I hereby authorize any licensed medical personnel to perform any accepted medical procedure deemed necessary and agree to bear the expense of any such treatment. I hereby affirm and acknowledge that I have read this document. I hereby affirm and acknowledge that I understand its contents and agree to be bound thereby.
