The student or parent/guardian understands the risk of studying Martial Arts and hereby releases Sunnyvale Martial Arts Academy, all instructors and all other students of Sunnyvale Martial Arts Academy from any and all liabilities for any type of injuries or loss sustained while training, studying, practicing, or in the application of Martial Arts (onsite or online).  You also state that the participant is in good physical health and knows of no reason why he/she cannot study and participate in Martial Arts.  The participant understands that Sunnyvale Martial Arts Academy does not offer refunds.  In the event of an emergency, I hereby authorize any licensed medical personnel to perform any accepted medical procedure deemed necessary and agree to bear the expense of any such treatment. 

By completing this waiver and attending class, I agree to follow instructions given to me by my instructors and others at Sunnyvale Martial Arts Academy to wear a mask, to maintain proper social distancing, and to clean my hands, among other steps to reduce the chance of catching or spreading COVID-19. I do not have any symptoms related to and have not, to my knowledge, been exposed to COVID-19. (Examples include a new cough, headache, weakness, fever, difficulty breathing, etc.)  I also voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending classes. Even if my symptoms appear to be mild, it is possible I could infect others whose symptoms could be much more severe. It is possible I could become infected due to actions, omissions, or negligence of myself and others, including, but not limited to, employees, volunteers, and other students and their families. I understand that such exposure or infection may result in illness, permanent disability and death. By training at Sunnyvale Martial Arts Academy, I waive any claim I might have against the school for these risks

I hereby grant Sunnyvale Martial Arts Academy (SMAA) permission to capture and use pictures and/or video of me and/or my minor child(ren) while participating in SMAA activities for promotional purposes, including brochures, social media, and other publicity. I release SMAA and its representatives from liability for any personal or proprietary rights connected to the use of such photographs or videos. SMAA will only capture images or videos in public settings where there is no reasonable expectation of privacy. I understand that neither I nor my minor child(ren) will receive any compensation for the use of such media. This release applies to all future participation in SMAA programs for all signatories and the minors on whose behalf it is signed.

By registering for SMAA introductory special and programs digitally, including clicking an “I agree” button, I agree that this constitutes a legally binding signature and acceptance of all terms and conditions outlined in this agreement, with the same effect as a physical signature.
